A clear scan or photograph of the whole card. Corners cut off are rejected by the department.

Both sides, and linked to a mobile number you can receive an OTP on.

Rent agreement or ownership document for the place where food is handled.

Passport-size photograph
Of the proprietor or authorised person.

List of food products handled
What you manufacture, store, distribute, or sell. This decides which licence tier applies.

Manufacturers only, where water is used in the process.
